Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure we obtain important background information, like your medical history, and give you time to get to know your doctor.
What to Expect at Your First Visit
On your first visit, your provider will review your medical history and evaluate your health. We will review any necessary imaging and medical records and we will go over your goals and treatment expectations. Our team will be happy to answer any questions you might have. We DO NOT prescribe medications to our pain management patients on the first visit.
Please be prepared to provide information related to your problem.
If you are receiving our services through insurance a physician referral is required, they can fax a referral over and we will contact you to schedule an appointment.
If your problems result from a Workers’ Compensation claim, prior authorization is required.
Please bring your insurance card, drivers license, and updated medication list, along with prior imaging to your appointment.
